The coin has a flat bottom edge, has Ultra High Relief thanks to Smartminting Technology and has Antique Finish quality. It comes in a case, along with the Certificate of Authenticity. The reverse of the coin shows a three-dimensional representation of the Leaning Tower of Pisa, UNESCO World Heritage Site since 1987. In the background, a close-up of the beautiful tower openings. All around, the inscriptions: “LEANING TOWER OF PISA” – the name of the coin, and “1372-2022”. The tower is shown on both sides of the coin in Smartminting Ultra High Relief and it also has a flat bottom edge to allow a vertical view of the coin.
 
The obverse of the coin features the Tower of Pisa and an image of Pisa Cathedral. On the left, the effigy of Her Majesty Queen Elizabeth II; all around, the inscriptions: “ELIZABETH II” – the name of the Queen, “COOK ISLANDS” - the issuing country and “250 DOLLARS” – the face value.
